Additional seat for accompanying persons is subject to confirmation after the registration closing date and will be allocated on a first come, first served basis.
Only one (1) accompanying person is allowed to join. The accompanying person must be a family member and is restricted to parent, spouse, child (must be at least 18 years of age), brother or sister (must be at least 18 years of age).
Please seek approval from Great Eastern Management prior to registration.

I (the accompanying person) hereby wish to inform that I will not hold Great Eastern Life Assurance (Malaysia) Bhd responsible for any claims / damages / loss, injuries & etc, sustained during the trip. / 本人（随行人员）特此声明，在此次行程期间，如发生任何索赔、损害、损失、受伤或其他相关情况，本人不会要求 Great Eastern Life Assurance (Malaysia) Bhd 承担任何责任。*

DEVIATION (OWN ARRANGEMENT) / 后续行程 (自行安排)

For any deviation request involving a change to the return date and/or departure city, a deviation fee of RM350, plus any applicable airfare difference, will be charged.
Payment must be made directly to CT Planners Holiday Sdn Bhd by the specified deadline.

如有任何后续行程，包括更改回程日期及/或出发城市，将收取 RM350 行程变更费，另加任何适用的机票差价。
相关费用须直接支付至 CT Planners Holiday Sdn Bhd，并于指定截止日期前完成付款。

Notes:

For the accompanying person fee and single room supplement, please make payment to CT Planners Holiday Sdn Bhd upon submission of the registration form and by 27 February 2025.

• Seats are allocated on a First-Come, First-Served basis (with payment cleared).
• Please email your bank-in slip to us with your NAME, CONTACT NUMBER and DEPARTURE DATE clearly indicated.
• We reserve the right to adjust the charges for accompanying persons or single room supplements based on the final confirmed venues, airlines and activities.

TERMS & CONDITIONS / 条款与细则*
1) After registration, if the qualifier does not turn up on the day of the trip or cancel the trip or is prevented to travel for whatsoever reasons, a penalty charge of 75% of the trip ‘Final Actual Cost’ will be imposed on the qualifier. This penalty charge will be offset via the commission account of the qualifier concerned. Appeals for waiver of charge will be consider for cases with serious medical ground where an official proof from the medical doctor (registered medical practitioner) is required, subject to final review by Company’s in-house medical doctor.
报名后，如达标者在行程当天未出席、取消行程，或因任何原因无法出行，达标者将被收取相当于行程**“最终实际费用（Final Actual Cost）”75%**的罚款。该罚款将从相关达标者的佣金账户中抵扣。对于因严重医疗原因而无法出行的情况，可申请豁免费用，但必须提供由医生（注册执业医生）出具的正式医疗证明，并须经公司内部医生最终审核。

2) Please note that if qualifier’s registration form, passport or any other necessary documents are not submitted within the stipulated deadline, the appointed travel agency has the right to cancel the individual’s booking and no refund/appeal will be entertained.
请注意，如达标者未能在规定期限内提交报名表、护照或任何其他所需文件，指定旅行社有权取消该达标者的预订，且不接受任何退款或豁免申请。

3) Any incidental or side order charge incurred (other than that stated in the Company’s approved group traveling/itinerary) during the trip will be borne by the qualifier.
在行程期间，如产生任何额外或附加费用（公司批准的团队行程/行程安排中所列费用除外），相关费用须由达标者自行承担。

4) All travel arrangements will be handled by the Company’s appointed travel agency and qualifiers MUST abide by the rules and regulations set by them.
所有旅行安排将由公司指定的旅行社负责处理，达标者必须遵守旅行社所制定的相关规则及规定。

5) Qualifiers who travel on the trip are encouraged to purchase a Travel Insurance on their own.
参加此次行程的达标者，建议自行购买旅游保险。

6) Company will not be liable for any claims, losses, damages, injuries, etc incurred during the incentive trip.
对于在此次奖励旅行期间发生的任何索赔、损失、损害、受伤或其他相关情况，公司概不承担责任。

7) IIn the event of any request for changes/cancellation upon confirmation, the cost of the trip would not be reimbursed. Please note that if you do not submit the registration forms, photocopy of passport, etc within the stipulated deadline(s), we reserved the right to cancel your booking and there will be no refund of fees paid.
如在确认后提出任何更改或取消行程的要求，已支付的行程费用将不予退还。请注意，如您未能在规定的截止日期前提交报名表、护照副本及其他所需文件，我们保留取消您预订的权利，且已支付的费用将不予退还。
DECLARATION / 声明*
The personal information that you have provided in this form may be used, recorded, disclosed by or on behalf of CT Planners Holiday Sdn Bhd for the following purposes:
· Processing your trip registration form
· For administration and trip management purposes (air travel arrangement, hotel accommodation, transfer and etc).

The information that you have provided to CT Planners Holiday Sdn Bhd is necessary. If you do not provide CT Planners Holiday Sdn Bhd with the information, CT Planners Holiday Sdn Bhd may not be able to process, administer or manage your trip registration.

CT Planners Holiday Sdn Bhd may disclose and/or provide your personal information to the following parties for the purposes stated above:
· Third party service providers (who provide administrative or other services to CT Planners Holiday Sdn Bhd in connection with the operation of our business) including those located outside Malaysia.

您在本表格中提供的个人资料，可由 CT Planners Holiday Sdn Bhd 或代表其行事的相关人士，为以下目的进行使用、记录及披露：
· 处理您的行程报名表；
· 用于行政及行程管理，包括机票安排、酒店住宿、接送服务等。

您向 CT Planners Holiday Sdn Bhd 提供的相关资料是办理上述事项所必需的。
如您未能提供相关资料，CT Planners Holiday Sdn Bhd 可能无法处理、管理或安排您的行程报名。

CT Planners Holiday Sdn Bhd 可基于上述目的，将您的个人资料披露及/或提供给以下相关方：
