Providing personal data requested here is entirely voluntary. However, if I choose not to provide the requested information, CT Planners Holiday Sdn Bhd (CT Planners) may be unable to fulfill its obligations to me or achieve the purposes outlines below.

I acknowledge, agree and give consent to CT Planners, along with any third-party service providers appointed by CT Planners on its behalf to collect, store, use, delete, disclose, transfer, or otherwise process my personal data (provided now or obtained later) for the following purposes:

a. Handling the registration process for this trip or event, including but not be limited to organizing arrangements and administrating itineraries for CT Planners’s activities;

b. Undertaking administration tasks, which include but are not limited to process registrations for company-related activities,

c.  Sharing updates or communication on promotions and training;

d. Ensuring effective communication for any of the purposes mentioned above; and

e. Executing tasks relevant to CT Planners's role and responsibilities as principal for its advisors.

For these purposes, CT Planners may disclose and share my personal data with individuals or organizations, including entities within CT Planners's group of companies or other third parties, such as travel agencies, airlines, transportation providers, hotels and ground, insurance providers, and etc. Some of these third parties may be located outside Malaysia, requiring cross-border data transfers.

Consent granted herein also applies to sensitive personal data, such as health or medical information, to the extend necessary for achieving the purposes stated above.

I understand that unless it significantly impacts CT Planners's ability to fulfill its obligation or contradicts the purposes for  which I provided my data, I may revoke or modify my consent by notifying CT Planners in writing.

In case where I provide CT Planners with another individual's personal data (“Data Subject”), I confirm that I have informed the Data Subject and obtained their consent to disclose their data to CT Planners. I further certify that the Data Subject has understood and authorized CT Planners to process their data in line with the details outlined here.

I further confirm that I have explained the contents of the declaration above to the Data Subject, and he/she understands, agrees and authorises SLMAB to deal with such personal data according to what is stated in the declaration above

The Company provides no warranty or guarantee concerning any incentives and assumes no liability directly or indirectly, for issues arising from or related to these incentives.

If required, the Company reserves the right to modify itineraries, travel arrangements, accommodations, or other arrangements. Additonally, the Company maintains the right to cancel or revoke any bookings made on behalf of passengers.

I declare that the information submitted in this form is accurate and true. I understand that submitting false, misleading or incorrect information may result in liability.

I hereby grant authority to CT Planners Holiday Sdn Bhd (CT Planners) to share my registration information with relevant parties to meet its obligations. I also acknowledge that CT Planners reserves the right to amend submitted information if necessary and will promptly notify the organizer of such changes.
